Apologies if this has been asked before.

I would like to know if there is a way to setup a form where a customer completes the fields/questions on Page 1 and then it is forwarded to an internal department employee who completes the fields/questions on Page 2 and then it is forwarded to another internal department who completes the fields/questions on Page 3 along with an approval and then the form is complete. A notification email is also sent at each level.

Is this possible and if so, how can I set it up?
