Help!

Help!

Hi. I need to create a form and store it, and then email people the link. Then I want each person to open up the form, fill in information, and "save" as a new document, naming the document with their name: "smith, john" or "jones, suzy," etc. 

How can I do this? I open the document, make changes, but it only lets me save it back into the folder as itself. Thanks for any help.
