Is it Possible to display an image that was uploaded to a form in document merge?

Is it Possible to display an image that was uploaded to a form in document merge?

On my form I've got the "upload an image" field where patients upload a picture of the back of their medical aid card instead of typing out all their dependants.  

The process should go as follows:
  1. New patients completes form and uploads a picture of their medical aid card - picture 1
  2. Picture is then saved to work drive - picture 2
  3. Writer merges all the info provided on the form onto 1 page ready to be signed by the patient with zoho sign - Picture 3

However instead of point three happening smoothly the image won't pull through from the form (now saved to work drive) - picture 4

Please advise what must be done to make that image pull though without having to do it manually
