Job Quote Form

Looking for help to create a Job Quote form. Top portion of each Job Quote includes basic info (Customer Name, Date, Job Name). For the next portion of the Job Quote I want to be able to add rows on the Quote which includes: item, quantity, price. To add new items/rows, the user should be able to select "add new item" or "finish quote" when no additional items to be added.
