Mail Merge Time Field is Converted to 24 Hour Time

I have a mail merge that occurs when a form is submitted. The merged information is date and time. The date filed populates in the format I want however the time is converted to 24 hours time. On the form it is 12 hour time. 

How do I format the merge field on the Writer doc? Or, what is a solution?
