I am looking for a form that will allow me to do the following. Form is set up for Company A There is a Message to: Box which is drop down, and you select the name of the person the message is for. Then there will be an email box. this will autmatically fill in when you select who the message is for. This email address will then be where the email is to be sent to with all user content. Simply the form will have the following fields: Message For Email Address Caller's Name Telephone Number Message User inputed data shoudl be sent in the email.
