i want 2 input forms: the first will have 4 inputs: date / first name / second name / code this will result in a table from which the next form will take some info: the second input form will have 4 inputs (the first 3 will be drop down boxes with data from the results table above): date / first name / second name / number value the results table can be very plain sorted by date, then number value, then first name, then second name
