Why Aren't my Fields Mapping from Zoho CRM into Forms?

I have 4 CRM fields that will not map into a linked form.  2 fields are one-line text and 2 fields are email addresses.  Both the Account Module fields and the Form fields match in type.  Single Line:Single Line; Email Address:Email Address.

What am I missing?  Other fields map just fine.
