Age Banded Pricing

I am trying to setup a way to price a membership based on age banding and household size. Example: Ages 18-29/30-49/50-64 single, married, family will all have different pricing variable.
I do not think it can be done in subscriptions alone. Is there a way to use forms or creator to filter out results or age and household size, and then land on the embedded widget that I would create in subscriptions? I do want them to signup in subscriptions in order to manage billing. 

If someone has an example age they could link to that would be extremely helpful.
