A database to record anonymous patient data for a clinical study on a wound healing product. Need to record patient baseline details, overall health, which practice they belong to and initial wound assessment to start. Ideally the system would create and assign a unique patient identifier. 

When treatment starts, need to record on each visit an update on that patients wound condition, who treated and the wound details including option to upload a photo. Treatments will be two per week for a nominal three week course followed by a patient review at which point treatment may be stopped, continued or modified Need to be able to produce output of healing progress per patient and then various statistical outputs also - but records MUST be associated with patient. Ideally inputs made at treatment via smartphone
