Please see the image attached. The current form (part filled in as example) is filled in by hand. I am looking for someone to make this into an online form. All the items surrounded by boxes are the variables that need to be inputted. Ideally I would like part of my form to include the background image of the t shirts, where text could be entered, to describe print requirements.
