Fix Search Script for Database

I am creating a database of products but I am having trouble creating the search form for it. I think it's an easy fix if you know Zoho programming. I have another form just like it that works fine. The database is called Green Product Directory. I need the search records view fixed so that an end user can search either by drop down category or by product name.
