Need help getting stateless form to work

Need help getting stateless form to work

Using the Donor Management database as a template, I am trying to make an underwriting database for our non-profit. I need to be able to let volunteers edit one set of records using a stateless form. I have tried to get the stateless form using the Zoho example but it will not work.
