We need to set up a database to collect demographic data of Ophthalmology Residency Programs around the world. We need 3 forms: One to collect new data. Should have a field that would allow to be seen in public listings. One to edit data by us One for users to request editing data (we would update, ideally we should be able to click and replace the old data) We need a report that would list the records as a table showing some of the fields. One public and one for us Should have a default sort but allow sorting by any field
