SalesIQ add Lead button not working,

SalesIQ add Lead button not working,

Currently trying to use salesIQ and integrate it in CRM, salesIQ was installed successfully, apparently, add Lead button is not working properly, add Contact is working tho.

the form for addLead is submitting, and 200 response, but it is not firing, some javascript errors is logging in the console. attaching screenshot of the errors,
