Save Zoho Creator APP Data in Zoho CRM Form

Hi!

I created a purchase requisition application in Zoho Creator.
I want when the user adds a requisition that requisition is also saved in the Purchase Orders form in Zoho CRM.

My Zoho Creator application has a subform with 2 fields. These are Product Name and Quantity.

I need the 2 fields of the subform to connect with the subform of the module.
