Setup an automation for Membership start date and end date

I am looking to create an automation or work flow that if a membership start date exists , it should automatically set an end date for after 16 weeks ,but send a reminder to me on Slack at week 14 , to talk to a client.

It would be great if someone can help out as soon as posible.
