Upload file/Submit Essay

We'd like to queue student submissions and create them as Cases in our CRM so that they can be linked to Student accounts (already created) and also provide grading on a 1st come first serve basis. The form would allow students to enter their email, name (first, last), a drop down selection for the assignment title and then a "browse/upload" option, then Save/Submit. The save/submit action would create a case in Zoho CRM with the appropriate fields already filled out.
