Visitor Summary Not Populating in CRM leads

I have integrated Zoho SalesIQ with the Zoho CRM leads for visitor tracking as well as added the Javascript API onto the website. I am currently using jotform forms that have the Zoho CRM integration setup, but when a new lead comes in via form submission on the website, it does not populate the visitor summary.

How can I get the visitor summary information to populate in the CRM lead information when a new form is submitted?
