Add custom field to hosted pages?

Hi! I'd like to collect more information from subscribers when they sign up for a subscription through hosted pages (for example, what school they went to, their t-shirt size, etc.). Is this possible? If so, how do I do this?

I see how to create custom fields attached to customers, but those custom fields don't show up on the hosted pages.
