Formatted resume - please help its been a year and still cant get it working!@!!

ive created a template - and now finally have the template actually appearing as it should. As I normally send to clients. 

However i cant get anything to pre-populate, Nothing, no names, country of origin, expected salary, resume with no contact details. Ive followed instructions exactly, however it just does not pre-populate the details. 

please help..... i was hoping Zoho would save time and money with this feature however it is not..... 

Any suggestions? ? ?
