Reporting on Cancellation Reason and Notes Fields

Reporting on Cancellation Reason and Notes Fields

I'm trying to find a way to generate a list of cancelled subscriptions that includes the cancellation reason and the notes field from the subscription and am running into a road block.    Here is what I have tried:  
  1. Reports in Zoho Subs - can get reason from churned subs reports, but no notes.
  2. Zoho Analytics - can't seem to get either field
  3. Zapier - reason and notes doesn't seem to be available
Any ideas on how I can get this data?
