I'm following the instructions from here :

But I can't get this to work. Here is the result of a test booking :

I'm specifically looking at the fields Company Name and Job Title (and I'm aware that the comment field is a long one, but that's what was requested !)

I have setup the below

Making sure to use the double underscore for the spaces

But when I view the inputs it just says null
