Hi,

I want to create a questionnaire that produces a final score upon the responses of the user.

For example, a question might be:-

"Do you eat potato chips?":

     Sometimes
     Rarely
     Never

I want to apply a score where Always=4, Sometimes=3, Rarely=2, Never=1

There will be more questions like this, and at the end I want to add them all up and provide a final score. 

Can this be done with Zoho Forms? If so, how?
