Displaying Data on a Form

I have data imported from a spreadsheet that I would like to display at the top of a form. It is to be referenced prior to the person filling out the form. Is there a way for me to import data into the form and then have them add the content and save the form with the data being added to that record???

Example: We are doing a round of phone calls. I would like the caller to be able to reference the person's name and information before making the call. Then when they make the call they will be able to fill in the call outcome.
