The form I created appears great in the form editor (aligned in two columns) and I like this layout (see the left image). 

But when I publish my website, the form only appears in one column so it goes as a long long list (see the right image).

I want the form on my website to be aligned into 2 columns just like in the left image. How can I achieve that?

I'd like to make another form that is similar to the existing form (I just want to change a couple of the fields and keep the rest as they are). 

Is there a way to duplicate the existing form and then edit? I don't want to make another form from scratch in the form editor...
