Issue with sending a Form via Campaign

Issue with sending a Form via Campaign

I created a fairly simple Form in Zoho Forms...it's for a giveaway (enter to win). I just want to send the Form itself to a segmented list in Campaigns from my CRM and when I try to do so, it still asks for Content. I thought the Form was the Content...what am I missing? I just want the form to be the actual "content" in essence.
