Send the most recent Note added to a Lead

I want to be able to email important Lead data along with a message to a Sales Rep. The Sales Rep is a field I've added to the Standard Lead Page Right now I have this partially working with a workflow and email template. The problem is there's no way for me to add an extra note.

What would a function look like that would work like send email button that was addressed to the email of the Sales Rep and include details such as the Lead's basic contact information?
