Silly question .Customer Inquiry form submission

I have an application in ZOHO Creator to  track clients and Service providers
In my application I have a form with the following fields for customers to complete once they have used one of my recommended suppliers

Your Customer name
Your Email
Facitlty Attended
Date attended
Star rating

I want to be ale to have this form available to any one in the world and have them be able to submit the form, and the data automatically show up in my application, which is private only to my Customer Service Reps.
Is this possible to do with Zoho Creator?
