Hi i had made a lovely form for customers to leave reviews for various members of our association. The idea was a customer would use the lookup field to find the relevant member, review them and the data would come back and be stored on the crm. We do need the customers details. I understand this is not going to work in a publicly shared form.

I guess i could do a form for every member but that is a pain. 

Is there another way. The idea is our member could give them a link or even a QR code.
