web to lead not working

Hi - I hope you can help as I can not seem to find my problem,

I am trying to generate a web to lead form on this page of my website:

It is generating on the page but the information is not going through or generating an alert email.

I have set domain to * but still not working.

I am sure it is something simple - can you help
