Why is zoho asking for viewers to sign in to an Zoho account where my contact form should be?

I built my two contact forms in Zoho creator. When I logged onto my webpage, zoho removes my contact form and asks viewers to sign in to zoho as if they have an account with Zoho. How can I fix this? Example is given of what viewers of my website see:
