Zoho form Feeding inforamtion to CRM

I have a zoho form that comes from our website that populates a contact record in the CRM. Some of the notifications are being sent to me but not getting populated in a new record in our CRM. The form asks for an email address, but if left blank it will not populate a new record. Can you help me, please?
