I have a simple form.  I have 3 required fields/sections.  Rest are hidden.  I have a subset of questions I'd like to have answered via a dropdown.  But no matter what I do I get 'error occured.  Enter a proper value"

For example, I had radio buttons (changed to drop down to see) with a yes/no values.  If they choose 'no' - then I want show unhide/show another question and mark them as mandatory.  

Just something this simple and I get that error with no other way to figure out what it doesn't like.  Any ideas?
