2 questions on forms:

we are working on this form

1) the submit button at the end of the form does not produce any result.. it seems it's not working. I checked the help center but no help.. sorry

2) I cannot find a way to create a filter when sending out the form to customers. I would like to have and give out a password and not an www open url.
