Embedded form on website not working?

Embedded form on website not working?

I have the embedded script in my website, and have verified that it has been copied  & pasted correctly (a number of times). However, after two tests, I get no notifications and can not see new activity in my reports. The page link is http://harrisoncontracting.com/contact/

Any idea why this is not working?  Should I use a different embed method - this is a WordPress site?
