How to embed "Zohoforms dialog html" in the Home page

Help me Guys

I want to have a "disclaimer" displayed in the home page, one shall read and agree by clicking the button on the form, before one reads contents of my website.

I intend to use a Zoho form Pop up dialog HTML.

My question.

1. I have generated the HTML, but want to know where I shall embed the same, so that the same is featured in the home page.

2. Any other idea than a zoho form, please suggest. Someone suggested a flash image which will give an exit option also.

Please help.
