Invalid Entry on a simple form

Invalid Entry on a simple form

I have a 10 question, multiple choice survey.  Each question requires the user to select 1 choice.  However, no matter what is selected, the form says "Invalid entries found". See below.  I have check each and can find nothing wrong. Please help.
