Updating contact and company details in CRM via forms

We want to send our contacts a mail on a yearly basis through which they can confirm or change their details (including company name). This is important as our clients are educational professionals, who often change school or oganisation yearly.

We have created a personlised form which our contacts can review and use to change their details.

When they change the company name, a new company is created in CRM however
- their contact details are not linked with the new company

How can this be solved
