Zoho CRM Webform to add activity to Calendar

Is it possible to create an activity in the calendar from or at the same time as a Webform?

Example

Mr Alex Nicol Lives at 123 Main Street, Town, County, PO12 3AB - (Already done)

And is booked for an appointment on the 30th February 2045. - (Date Picker, Which is added to the calendar and even assigned to a particular person.)?
