Customize submit new ticket form in customer's portal

Customize submit new ticket form in customer's portal

Hi, I'd like to know how to customize the submit new ticket form in the customer portal. I want to add the option to select the ticket's due date. 
I know I can customize the portal's home screen (HTML and CSS) but I could not find the option to modify the submit new ticket form.
