How to add text to a notebook or card from my Apple Watch 8?

How to add text to a notebook or card from my Apple Watch 8?

Hi—I can see how to add text to a card or notebook from iPad or iPhone and read it on my Apple Watch 8,  but not how to add text to an existing card or notebook from my watch. Writing from my watch always seems to create a new card, which then appears on my other devices. 

Thoughts?
