To convert document images into an online form:
Click New Form in the top-right corner of your forms listing page.
Select Images to Form to proceed.
In the Convert Images to an Online Form popup, drag and drop images or click Upload to add them.
If you want to add more images before conversion, click + Add Image in the preview screen.

1. Supported image formats are JPG, JPEG, PNG, BMP, WEBP and JFIF.
2. The maximum upload size supported per image is 5 MB.
3. A maximum of 5 images can only be uploaded.
Once all the required images are uploaded, click Convert to Form.
Zia AI analyzes each image, identifies form fields, and generates an online form.
The fields identified from each image are placed in sequence one after the other within the same form.
Your form will now open in the form builder. You can modify, add, or delete fields; change the theme; and configure rules, integrations, and workflows.

Choose the right option based on the type of file you have:
Use PDF to Forms when a document or form is already available as a PDF file.
Use Images to Form when you have to convert a paper form or a digital form quickly to an online form. Capture a photo or take a screenshot, and upload it to create the required form instantly.
