Annotate widget?

Is there something in creator or any zoho app that allows me to have an image markup field item in the form?

I need to be able to complete a form that also allows the user to mark up a preloaded image. Other compay's call this an image markup field or an Annotate image widget.

Use case is veterinary dental exam chart/ form. During completion of the form, need the ability to annotate a dental graphic
