E-sign

I added a document to be filled out with a player name, parent name, address, signature, date.  I want to be able to send the document to multiple people.  Currently when you go to fill out the form, the same name will be entered into all of the name fields.  I want to have the fields labeled, for player name, parent name, email, signature etc but I want to be able to send it to multiple people at once.  Plus I dont want the name to be entered into all of the fields.  How do I fix this?
