Emails to be sent out at different stages

Hi, Please can I have some tips..
I am trying to send 4 emails (1 every 3 months) containing a survey through campaigns. 
I have an autoresponder set up based on a date field having being a certain number of days old, but it doesnt look like anything is going out. Is this the best way or shall I try something out?
