Form templates vanishing

I'm just getting started with campaigns and my form templates keep on disappearing from my library.

I first set up a form, and copied the code over to my website, then the next day it was gone. Assuming I'd made a mistake, I created a new one, and copied that code over. After testing the flow and editing the responses on the form, I found I had 2 duplicated forms that I did not create. So I deleted those and finalised my responses, tested it was working on the site and then my form library was empty again.

So I technically have a working form on my site, which suggests the form's data still exists somewhere, but I can't access the form in my library to edit the responses if I need to in the future, so it feels like this is an almost unusable workflow. Has anyone else had issues like this and/or could there be anything that I am doing to cause this
