I would like to QR code scan to edit a form entry. 

I would like to take the URL of a submitted form entry and create a QR code with the URL. Once we have the QR code we would like to print a sticker with the information and QR code. We would then like to be able to scan the QR code that takes us to be able to edit the entry of the submission.  Any help on how to set this up would be greatly appreciated.
