How can I create an event attendance contact info and customer waiver signup?

My small business is hosting an event in the near future and I want to get contact information and liability waivers signed by incoming visitors on a tablet.

It is important for the customer to do this self-service style, rather than have myself or one of our staff manually typing information, because we will be controlling traffic flow. 

This is something completely new for me to try with Zoho because I have only ever used it for internal operations. As basic as it is, I'm at a loss of how to go about it. I Flow should look like this:

1) Customer grabs tablet with blank fields
2) Customer fills out contact info, address info, email/phone
3) Customer is taken to a zoho sign liability waiver (from template), signs it
4) Zoho creates the new Lead record and associates the waiver with it (through Zoho Sign integration)
5) Zoho returns to empty screen for next person

I vaguely understand that wizards or kiosk studio may work, but kiosk studio looks to walk a staff member through a work flow, and that isn't what I want. I've never used anything functionality like those features.

Can someone help please?
