There is an option for demographics where you can select fields such as "Name", "Phone", etc. that we want to use in a survey.  However, with this survey question type, there does not seem to be a way to include a title/caption.  What we are wanting to do is have a statement along the lines of "Please type your name and e-mail address below to be added to our list serv" and then have a field for "Name" and "E-mail".  We can't figure out where to go to add a caption (the sentence saying "please type your name..."). 

Can someone provide us direction, please?
